APPLICATION PROCEDURE:

If you are ready to proceed with the application for an IBC, Trust or Ship/Yacht Registration, please complete the necessary application forms as detailed below and send them either by email (to orders@bosl.com) or by fax (to: +501-822-2989). Please ensure that all forms are completed in full and duly signed where requested and are accompanied by the necessary supporting documents.

International Business Companies (IBC’s):

• IBC Application Form
• General Indemnity Form
• Declaration from Directors, Shareholders and Beneficial Owner
• Accounting Records Declaration
• Request for Office Services Form
• Due Diligence Documents (as specified below) for each Director, Shareholder and Beneficial Owner

Trusts:

• Trust Application Form
• General Indemnity Form
• Declaration of Source of Funds
• Due Diligence Documents (as specified below) for Settlor, Protector and each Beneficiary

Ship and Yacht Registration:

• Fee Request Form – Ship and Yacht Registration
• Due Diligence Documents (as specified below) for Owner and Ship’s Representative

Due Diligence Documents:

a)     For each individual person, please provide:

• Certified copy of valid Passport or National ID Card (photo ID and data page)
• Original or certified copy of a Utility Bill (issued within last three months)
• Original Banker’s / Attorney’s / Accountant’s Professional Reference Letter

b)     For each corporate entity, please provide:

• Certified copy of Certificate of Incorporation
• Certified copy of Memorandum and Articles of Association or By-Laws
• Certificate of Incumbency confirming current Directors and Shareholders
• Certificate of Good Standing
• Certified copy of valid Passport or National ID Card of each of Director, Shareholder and Beneficial Owner
• Original or certified copy of a Utility Bill (issued within last three months) of each Director, Shareholder and Beneficial Owner
• Original Banker’s / Attorney’s / Accountant’s Professional Reference Letter for each Director, Shareholder and Beneficial Owner
• Declaration Form from all Directors/Shareholders/Beneficiaries
